Getting the most out of failures

    What will the event cover?

This talk will discuss what we can learn from analysing weld failures to help us avoid them in the future and prevent problems with operational plant.

    What will you learn?

Failure investigation is only a small, but essential part of the root cause analysis process.  This presentation highlights the difference between failure investigation and root cause analysis, considers the methodology of a failure investigation, and using examples, covers some of the pitfalls related to interpreting the data arising from failure investigations.  Some of the challenges associated with failures which occur frequently, or occurred due to repairs being carried out without investigation of the failure are also considered.

    Our industry expert:

Joanna Nicholas, Consultant Welding Engineer at TWI Ltd

    Meet the speaker:

Joanna joined TWI Ltd in 1999, and has been involved in research, materials selection, and failure investigations, mainly with ferritic steels, alongside welding consultancy.  She has also been involved in various aspects of computer modelling of weld phenomena and welding software supporting standardisation of weld procedures and welder qualifications.

Joanna has a broad knowledge of ferritic steels with particular emphasis on hydrogen cracking and hydrogen diffusion, HAZ hardness, mooring chains, fasteners, in-service welding and 9% Ni steels.  She has carried out numerous investigations in these areas including failure investigation as well as production qualification and research.

    Saving Lives - Submarine Rescue

    What will the event cover?

In the unlikely, but unfortunate event that a military submarine should become stranded on the seabed (DISSUB), it is likely the submariners on board are unable to return to the surface unharmed due to excessive diving depths in which conventional evacuation cannot be performed. A smaller, more versatile manned submarine is required to deploy and rescue the stricken submariners on board.

FET have been the market leader in the supply of rescue submersible vehicle (SRV) systems. Since 1975 FET have pioneered the development of manned submersibles. One previously delivered system includes the NATO Submarine Rescue System (NSRS) which entered service in 2008. NSRS is on constant standby and is rescue ready 24 hours, 365 days a year. Subsequent SRV deliveries include China and Vietnam and most recently the new LR600 SRV, awarded in 2025 from the Indonesian Navy.

Being a manned subsea vehicle, the design of a rescue submarine systems encompasses all elements of engineering excellence, including safety, code compliance and quality assurance. The talk will present the various elements of the design, manufacture, testing and trials currently being employed towards the new flagship LR600 Rescue vehicle.  The presentation will address the various elements within the SRV design being:

    • Lloyd’s Register Accreditation and Code Compliancy
    • Pressure Hull and Hatches
    • Lifting Appliances
    • Propulsion
    • Air and Oxygen
    • Life Support
    • Main and Emergency Hydraulics
    • Depth, Bilge and Ballast
    • Electrical
    • Fire and Safety

As a precursor to the SRV, an introduction to FET and its portfolio of subsea vehicles shall be presented.

    Our industry expert:

James Foster-Rain BEng (1st Class) CEng MIMechE

    Meet the speaker:

James Foster-Rain is the Engineering Manager at Forum Energy Technologies (formerly Perry Slingsby Systems Limited PSSL). He has served for 23 years within a mechanical engineering discipline.

Whilst at FET he has progressed from design engineer to senior engineer, project engineer and subsea tooling manager. Achieved Chartership in 2012 via the IMechE. In 2024 he was appointed Engineering Manager, managing a department of over 30 personnel of mixed engineering disciplines.

James has been involved in all areas of the subsea design, manufacture and testing for ROVs, trencher systems and submarine rescue vehicles. Whilst at FET he has participated full time on the flag ship rescue submarine systems, namely the NATO system. These projects have enabled James to travel and work abroad and participate on mobilizations and sea trials for NATO, the Chinese and Vietnamese Navies. During his earlier years at FET, James focused on mechanical design and structural engineering.

Prior to joining FET, James left school and joined the automotive industry working at Federal Mogul for 10 years, included 3 years in Germany. Whilst at Federal Mogul he served his mechanical apprenticeship and then attended University via the company sponsorship scheme. He achieved a First-Class Honours degree in Mechanical engineering then subsequently moved to Germany as a graduate mechanical engineer.

North Scottish Branch with Highlands & Islands Branch Annual General Meeting (2026) (Online only) and

Technical Presentation "Two Steps Forward and One Backward" - Reflections on 40 years of Welding &  Fabrication

The Welding Institute North Scottish Branch with Highlands & Islands Branch invite you to their AGM:

The AGM will take place on 29 January 2026 at 5.30pm (Online only).  Following on from the AGM there will be a technical presentation on "Two Steps Forward and One Backward" - Reflections on 40 years of Welding & Fabrication

    What will the event cover?

There have been many changes and improvements in welding and fabrication over the past 40 years. However, much of that is incremental, and a modern fabrication shop doesn’t look all that different to one from the 1980s. It seems that most of the innovation has been in the area of mass-production, where robots have taken over the welding, or welding itself has been replaced by adhesives and alternative joining techniques.

    What will you learn?

The proportion of automatic and semi-automatic welding has steadily increased compared to manual techniques (particularly at the expense of MMA electrodes), although manual TIG continues to dominate single sided root runs.

The use of computerised material handling and weld history systems has been well-established for at least 20 years, but documentation handover is still largely by PDFs in Manufacturing Record Books, though sometimes supplemented by separate data transfers.

The use of digital devices on the shop floor continues to increase, but incorporating the data from them is significantly more difficult today than it was 25 years ago, mainly due to the higher level of control required by IT departments to ensure security and data safety. Development of computerised management systems usually takes much more time than it would have done at the turn of the century, despite the availability of better tools and development platforms. Will AI help?

In some areas, we seem to have slid backwards rather than moving forward, but these problems can be solved by innovation and good management.
